The MVA intends to provide copies of most MVA forms which can be downloaded. Some are intended to be printed and filled out; others may be filled out on-line and then printed or emailed.

Here is a Microsoft Excel version of the MVA's form 3 (After Action Report). If you would like to download a copy of the form, click here MVA Form 3 (AAR). If you have Microsoft Excel, you can fill the form out on your computer, print out the filled out form, and mail it to the MVA treasurer.

If you don't have Microsoft Excel, you may download a .pdf version of the form, print it out and fill it out by hand, then mail it to the MVA Treasurer. To open the .pdf version, you will need Adobe's Acrobat Reader which can be downloaded for free from the Adobe web site, or Mac users can open it with Preview, provided with Mac OS X.
